This is the big distinction that I made in my "Packaging Asterisk for the 
SOHO Market" presentation at Astricon. There are two different models at 
play here: VoPI (Voice over the Public Internet) and VoPN (Voice over 
Private Network). The former is uncontrollable and a wildcard at best. No 
way to pass QOS information between providers, no SLAs etc.. The later is 
the much more intelligent and business friendly way to approach the 
provisioning of services, providing for private, QOS, Traffic Shaped, 
Redundant, Private Links between the Term/Org provider and the end-user.

While in general, I am not a fan of the hosted PBX model, if you are going 
to do it, make sure you do it using a VoPN model.
