As for the activation solutions they were developed to make sure cards
do not get stolen.
So in most cases that means that shop or point of sale 
selling the card has to buy the card first from the oprator.
Sometimes thay have special account with operator that is charged
when the card is purchased. 

I think that really neat idea is to use gprs and simple java based
application on mobile phone. You can get that pretty cheap with
corporate package with unlimited gprs when
you talk to your mobile sale representative.

With every sale cashier has to enter 7-10 digits on the phone and press send 
button and he'll get a response if 2-5 seconds from main server. 

Simple, cheap and efficient IMHO !
I have proof of concept running on nokia 6310i all you have to make
sure is that you have gsm reception and battery in the phone is ok
