Like CS said, it's illegal in most of the Carribean countries to offer
any kind of telecommunications service without a license from the
respective governent.
In some countries, you can't even get a license because Cable&Wireless
still has a complete monopoly.
In other cases, the license-fee becomes prohibitive for anyone not
extremely well-financed (such as other telcos!).
The license-fee to become an international telecom carrier for
Barbados is $500K (barbados dollars) or 3% of adjusted annual revenue,
whichever is higher.

(It wouldn't take much to undercut the .15 to .20 charged for calls to
Barbados...)
