We use an app called Omali

It isn't perfect - and the database is always getting stomped on by the
local ILEC - at least for the ported numbers - but it does work.

It's basically a portal service. We edit our portion of the E911
database and push it up to the portal. They push the info out to the
proper places. Updates take 24 to 48 hours.

Its up to us to get the calls to the proper PSAP... which turns out to
be somewhat easier than updating those damn databases. So I feel your
pain.

For some sites (areas not-covered by Omali) we are considering using the
local Fax line and hooking up an ATA to that line (with a cutout switch)
and using it as the local 911 trunk for that client.
