Progress report on LiveVoip. They switched our number from a San Diego 
gateway to a Van Nuys gateway and the packet loss situation is better, 
this is a usable discount-grade level of service. They still don't pass 
audio from Asterisk until the call is answered, so you will have to use 
Background, music on hold ringing or something else, regular ringing in 
an extension is not heard by the  caller.
