[Asterisk-biz] Looking for New York City Origination Provider

I have my own Asterisk box.

I'm looking for a 212 Origination Provider that

1. I can connect to via IAX from my asterisk box
2. Accepts American Express
3. Can port my existing 212 numbers
4. Has commercial type billing: that is minute billing and DiD billing
rather than one lump sum rate.

I've scoured around for these requirements and cannot find any providers
that fit these criteria.
