Further analysis showed that a call placed using a GTalk channel
which came as "Restricted" was not recorded under History / Placed
in Google Voice.<br>
<br>
A call placed using the same GTalk trunk an hour later was
terminated to the same recipient's phone with the proper CID.<br>
<br>
It looks like a call routing issue on the Google Voice end to me.<br>

Let me summarize:<br>
<ol>
<li>GV Outbound CID shows "Unknown", "Unavailable", "Out of
area" (depending on a recipient's carrier) starting some time
around 02/15/2011 if a call is placed via Google Chat/Google
Talk/Google Mail/Asterisk GTalk channel. See

href="http://www.google.com/support/forum/p/voice/thread?tid=49c21d292e80ff65&hl=en&start=40">http://www.google.com/support/forum/p/voice/thread?tid=49c21d292e80ff65&hl=en&start=40</a>
for other users' accounts.</li>
<li>This CID feature failure affects only some GV phones, some
still work fine as of 02/24/2011.<br>
</li>
<li>Calls placed with GV call-back facility work fine for the
phones affected by the issue described in #1.</li>
<li>A workaround is to set Caller ID (incoming) to " <label
for="gc-caller-id-radio-two" class="gc-label-align"> Display
my Google Voice number </label>" As expected it will
suppress an incoming CID. So it is not a perfect workaround.</li>
<li>Another workaround is to trigger a GV callback facility per
William Stillwell's posting. Connection time increases with
this workaround.</li>
</ol>
Historically it took 15 days to a month for Google to fix similar
problems.<br>
