unsolicited NOTIFY is in the case of a MWI. The reason that the unsolicited
NOTIFY is sent as a voicemail notification is that the vast majority of vendors
have implimented it in this way. very few devices can explicitly subscribe for a
MWI, so Asterisk uses the registration of the device as an implicit
