use func_odbc, create a new function that does a lookup.
[CALLERID]
prefix=LOOKUP
dsn=MyDB
readsql=SELECT CALLERID from MyNames where CallerIdNum =
'${SQL_ESC(${ARG1})}'
exten => s,n,Set(CALLERID(NAME)=LOOKUP_CALLERID(${CALLERID(NUM)}))
