Your own local Asterisk box behind it's static IP address. (or static 
enough to be usable)

Set it up to accept inbound calls to (e.g.) david at my.ip.address.com and 
give that to IPKall.. Then you can route the incoming calls where you 
like.

I never did understand FWD when I had my own asterisk box. (Although 
out-dialling to US 800 numbers was handy once or twice)
