What version of Asterisk?  I ask because I have had
Reports of problems against svn trunk and svn branches
after 1.4.0 was released.

These steps will help identify the problem-
1.  module unload app_cbmysql.so
2.  core set verbose 10
3.  core set debug 10
4.  module load app_cbmysql.so
