if on freebsd..

stop asterisk

killall  mpg123
cd /usr/ports/audio/madplay/
make && make install

edit musiconhold.conf

[default]

mode=custom
directory=/usr/local/share/asterisk/mohmp3
application=/usr/local/bin/madplay -Q -o raw:- --mono -R 8000 -a -12


then restart asterisk..
