i had a similar issue with the first branch of asterisk 1.2 and cheap
phones (tip-100 from tatung)
i'll suggest you to upgrade your asterisk box
are you using bristuff ?
try bristuff-0.3.0-PRE-1q which comes with asterisk 1.2.9.1
