I have a Norstar MICS and was unable to find a way to make it send out the
name as well as the number.
Workaround: Have each handset have it's own OLI, and then use Asterisk's
LookupupCIDName, or use a macro to do the lookups in the realtime voicemail
db and use SetCIDName.
