[Asterisk-Users] CDMA gateway

I will get a CDMA gateway, which connects to a FXO port of Asterisk.
The connection from the CDMA gateway to my CDMA is a flat rate!!!

I need some help in thinking about the dialplan:

I want that with each phone call will be also forwarded to the CDMA 
phone, however, only if I press a code, I will get the phone call, 
otherwise it should hang up again.

I want that I can call back, so I want to have my caller-id containing 
my CDMA gateway phone and the callers number, so that I can just call back.

I want that if I call my gateway, I want to dial my CDMA followed with 
the destination number. The system should aks me than for the passpword, 
if no password recognized, than only the office phone should ring.

Any ideas about that? Does such dialplan (maybe agi) exist?
