through asterisk

Have an Asterisk on a real IP collecting SIP calls through Internet and
passing them to Telecom Companies.
Some SIP clients are on a real IP, some others behind NAT.
The issue is, when SIP clients are calling each other, they use system
resources and bandwith of Asterisk server.
The question is:
1. Can Asterisk establish a P2P (w/o passing through Asterisk) call between
a SIP client on a real IP and another SIP client behind NAT?
2. Is there any efficient solution to place P2P calls between SIP clients
behind NAt?
Thank you.
Cenk.
