NPA-NXX does not correspond to a city/state. NPA-NXX correspond to
switches in central offices, known by CLLI codes. You might find what
your are looking for if you search for a CLLI code listing, the listing
should have the location (city state) of the the switch and the
NPA-NXX's it serves.

Be aware thought, that with local number poratability the NPA NXX can
actually point to any switch in the LATA and beyond with cellular and
VoIP.
