I set the Caller*ID before I place the outgoing call, like so: 

exten => _91NXXNXXXXXX,1,SetCallerID(IgLou Internet <5029663848>)
exten => _91NXXNXXXXXX,2,Dial(Zap/r1/${EXTEN:1})
