Do you have a [switch-2.nufone.net] entry in iax.conf?

I place calls by Dial(IAX2/myuser at nufone/${EXTEN})

I think if you're dialing it direct (i.e. no entry in iax.conf) you need to 
say ${EXTEN:1}@NANPA (i.e., specify the context).
