[Asterisk-Users] Quicknet Internet Phonecard

Can anyone post a set of instructions on how to install this card?  
 
I have RH9 Linux and the latest Asterisk.
 
I have tried to install the card using instructions from Quicknet and
the card works for a little while and then Asterisk issues a message
about the driver missing a fixup routine and then I get a repeating
error message from Asterisk about a "mini-frame errors" or something and
the whole Linux system goes into slow motion.
 
If I pop the card out, the messages go away, the system resumes normal
operation, and Asterisk seems to work.
 
If I push the card back in it is not recognized/recovered by Asterisk.
That only happens if I reboot.
