I usually block:

1900NXXXXXX
1976NXXXXXX
1NXX5551212 (except 18005551212)

and then, if you are allowing only US/Canada calls, you would want to
block all the other NANPA area codes (Jamaica, Trinidad, etc.)
one-by-one.
