How do I match a "D"? (Was: RE: [Asterisk-Users] In-band disconn ect problem (legacy PBX) - asterisk doesn't hear the touchtone?)

Some phones actually have 'A', 'B', and 'C' keys - a few even have a 'D' 
key.  Haven't seen any, but I know they do exist.
