your options are:
setup publishing rule on your IPtables to allow access to you internal * from external *
or:
register to the external server using the internalserver-peer username:pass
then you can, in your extension.conf, call dial(iax2/internal-peer)

Im using the other type configuration since the NAT server IP doesnt change..
On the External Server

How did you configure the trunk between the 2 servers?
using register => user:pass at external server
or using extension?
