What revision of TDM400P do you have? Is it REV I?
I have REV I and had the same problem before. I had problem when I connected the FXO port to the all plug using 4 wires phone cable. It turned out that the RJ11 port of FXO or FXS in REV I, does not work when pin 1 and pin 4 are connected to something. My problem was solved after I changed the cable to 2 wires phone cable.

I'm experiencing a very weird behaviour with my tdm400 with two fxo and one fxs modules. I setup my current configuration at home, I tried it and it works flawlessly. I moved the computer to my office and plugged the fxo to the wall plug, but when I tried to call I got a busy signal. I attached the same wire to a phone, I called again and the phone rang. I tried with both the fxo ports, but I always got a busy signal and on the CLI Asterisk doesn't notice the incoming call at all. Outgoing calls do not work either.
So I moved again the computer to the home of a friend, and it there it works too as it does at my place. When I plugged the TDM at the office no other phone was plugged in the whole structure.
I'm really puzzled and I don't know why it is behaving this way. Any hints?
