Yes, according to the document link that you provided and most other
sources, a T1 crossover cable is required to connect the Nortel Meridian to
an Asterisk server. Here is a summary of the important settings that I
have:

Nortel Meridian Asterisk Digium TE405P
--------------- ----------------------
PRI_CPE PRI_NET
Switchtype 5ess Switchtype 5ess
23B, 1D channel 23B, 1D channel
ESF framing ESF framing
B8ZS encoding B8ZS encoding
Get clock from span Provide clock

We still cannot get a connection. 'PRI SHOW SPAN 1' produces:

Primary D-channel: 24
Status: Provisioned, Down, Active
Switchtype: National ISDN
Type: Network
Window Length: 0/7
Sentrej: 0
SolicitFbit: 0
Retrans: 0
Busy: 0
Overlap Dial: 0
T200 Timer: 1000
T203 Timer: 10000
T305 Timer: 30000
T308 Timer: 4000
T313 Timer: 4000
N200 Counter: 3

It seems that the D-channel is down. Any other ideas or suggestions would
be greatly appreciated.

This might be an obvious question, but should you be using a crossover
cable?

Information on setting up Nortel to TDM card links can be found at:
