It's time to get lowlevel.

iax2 debug and look for "received DTMF digit '3'" or something.  tethereal 
will also show you the IAX2 IEs for DTMF. 

If you do not see this, the far side is not sending DTMF, and you need to 
complain to livevoip.  IAX2 DTMF is *always* out of band.
