[root at pbx root]# grep FAX_RX /etc/asterisk/extensions_additional.conf
FAX_RX = system
FAX_RX_EMAIL = foo at bar.com

These parameters are in the *_additional.conf file, not extensions.conf. 
Also they are under [globals] not [global].
