Ahh, the {EXTEN:1} must serve to skip the 1st character of the extension,
stripping the 9 off

I use NuFone, but here's how I do it (my area code is 207):
exten => _9NXXXXXX,7,Dial(IAX2/ausername at NuFone/1207${EXTEN:1})
