How do you force asterisk to use G.711?  Would this be necessary to
define in the FWD context in sip.conf?

I think your inbound failures may be a context issue, I had a similar
problem with the configs I got from you, I moved the extensions to the
from-sip context, and have been able to receive incoming calls since.

I have a sip debug dump from a failed outgoing FWD connection if that
would help anyone.
