[Asterisk-Users] Carrier Access Channel Bank Setup -- No hangup

Changes are below.  Use KewlStart for the FXO channels.  (Loopstart +
remote disconnect suppervision)  Define all T1 channels.  FXS channels
can be loopstart without any issues.
