[asterisk-dev] appdocsxml.dtd and see-also presentation

I have worked on adding the relationships (the see-also) to the
xmldocs.  I ran into two issues.  Adding "manager" to the xmldoc is
patched here [1] and can be committed.  The other issue is the
presentation of the see-also in the xmldocs.  If you look at a simple
patch [2] you will see that the see-also can have various
relationships.  I have tried many methods and hacked at the xmldoc.c
to present the see-also with multiple relations.

eg... MEETME_INFO is a function and MeetmeMute, MeetmeUnmute,
MeetmeList are manager actions.
