MWI is one of those weird cases where for quite a lot of users, there is no 
corresponding SUBSCRIBE transaction. Asterisk is configured to just send NOTIFYs 
to the phone.

However, I believe it would be pretty trivial to add the ability to store the 
To: header information from a SUBSCRIBE and use that by default if no overriding 
option is set in sip.conf.
