You really, really need to spend some time (weeks, if needed) learning 
what Asterisk does and how it does it before asking these supposed 
"development" questions. To answer your questions (although they are 
painfully simple):

- the module used to handle registration is the same module used for all 
other communication with that device: chan_sip, chan_iax2, etc.

- there is no need for you to do anything for Realtime ODBC support, 
it's already there in res_config_odbc
