Granted I am no where near a PRI expert, but it seems the remote side
complained about the digits you sent to it, but it is doing it inband.
Seems that asterisk sees that normal disconnect and is clearing the
channel and doesn't have the data to communicate to you what went wrong.

Quick suggestions is to make sure you are providing the correct kind of
number presentation and that you actually dialed the number as the
network is expecting it.
