I've had very similar issues using
Aastra390 <--> Dlink DG-104S <- MGCP -> * <- IAX[2] -> * <--> PSTN
Aastra390 <--> Dlink DG-104S <- MGCP -> * <- IAX[2] -> * <- MGCP -> DG104 <--> Phone
Same problems gsm/g711 fine with IAX spotty wity IAX2
