Patch Set 2:

I have changed the buffer sizes as discussed and rewrote the commit message.

Turns out matching the sample rate to the channel is really simple. I have added that to the code for Asterisk 11 (because that's the version I currently use). For the 13, 14 and master branches I will update the code within the next few days -- a simple cherry-pick won't do because the codec handling is different.
