I've posted another review [1] to document the issues and provide ways to avoid them.  That review has no effect on this one.

If we can get this one merged I'd appreciate it.  The other review resolves issues where the context is unprotected for very short times.  With func_periodic_hook the context is unprotected the entire time Asterisk is running.
