We "buy/rent" US toll free numbers at didforsale.com. We use SIP to get it
to our Asterisk server.

If I'm correct (I can look it up):
- 3 USD/month
- 0,05 USD/minute
- 5 USD setup

There aren't other requirements if I'm correct (like an address in the US).
