[asterisk-biz] 8 SIP Accounts request for prices

Hi,
For a customer small business I will need the following services.

Customer PBX runs asterisk 1.4.x + FreePBX

7 SIP accounts (g729 or g723)
Caller ID incoming and caller ID outgoing.
a phone number that be tied to the 7 acocunts, so potential buyers
only need to remember one phone number.
SIP accounts must be incoming/outgoing capable
If possible, that i can be able to select the phone number (area code+number)

1 SIP account (g711) for faxing purposes.
Caller ID incoming and caller ID outgoing.
SIP account must be incoming/outgoing capable
If possible, that i can be able to select the phone number (area code+number)

Payment methods
provider must take paypal or VISA, as I am overseas and not in the US.
I am a verified paypal account.
Also, I can leave a security deposit if needed.

Calling capabilities:
If possible unlimited US and Canada for a fixed fee.
All SIP accounts must have capabilities to make international calls to
anywhere, including cellphones.

Note:
Please provide verificable internet addresses to check your company,
comments, customer opinions, etc.
I am trying to avoid mom-and-pop providers

After reviewing the offers received, I will take action in 24 hours maximum.
