Media gateway
      (AS5300/Quintum/Digium card,etc. [depending on PSTN connection)]

      Asterisk is a good choice
Billing system
      Depends on customer base and business model
Administrative framework to provide DIDs
      (licenses, interconnection agreements, etc.)
