I am in no way affiliated with them, I havent bought their services,
but ...  clec.biz offers $35/lata flat rate tollfree in america.  If you
do 50 or more latas its $10/lata (so 15 latas is the same price as 50).
Note there are about 209 latas in the US.

The key to all of this is a trick, so here it is.  You *must* use a CLEC
who will act as the RespOrg for the lata in question.  The CLECs
alledgly arent billing for things that the ILECs do bill for (I am
waiting for this to blow up in someones face, although they claim it
hasnt yet).  clec.biz doesnt operate as a clec afaik so you are on your
own to find one that will and will hand the traffic off to you
(presumably via voip or t1/whatever).

If you dont have a clec in a given LATA that number cant be called from
that LATA.  If the CLEC in question doesnt peer with a given carrier
(should be fairly rare) then the call wont go through.  But if you are
looking at doing prepaid services or whatever that can be a good way to
have ak nown fixed cost rather than a per minute cost and can in some
cases be cheaper than a per minute cost.

The traffic is in effect handed off locally and never hits an IXC.  This
is more or less the secret to the rate.
