Broadvox has a carrier-level service with nationwide DIDs.... and a 
pretty feasible commitment of like only $1k a month

Check them out.  They do "virtual ds0's" also, where you have a virtual 
inbound/outbound channel and can throw as many DIDs at it as you want.
