Cleaned up the handsfree profile code extensively.
Changes:
* Rewrote do_monitor_phone() function to be more clear and more robust.
* Added locking throughout the code.
* Added debugging messages throughout the code.
* Tweaked hangup, answer, and dial handling.
* Hangup the chan_mobile channel when the bluetooth device disconnects.
* Cleaned up the mbl_pvt and the hfp_pvt structure.
* Tweaked output of mobile show devices cli command.
