Log:
* Move the smart bridge operation into the bridge thread.

* Refactor code to extract ast_bridge_channel_pull(),
ast_bridge_channel_push(), and bridge_array_grow().  As a result
bridge_array_add(), bridge_channel_join(), and ast_bridge_merge() are
simpler.
