Create branch for fixing timeout logic errors.

See issue ASTERISK-20414 for more details.
The idea here is to fix potential errors calculating timeouts.
I'm doing this in a branch since there are likely to be many
places this needs to be fixed, and doing this incrementally will
keep me sane.
