[Asterisk-video] H264 support in app_transcode

Hi everyone,

After a few months involved in other project I have been able to 
continue with my asterisk developments. The latest version of 
app_transcoder now supports H264 also in both decoding and transcoding.

I would like to thank Luis Guaman and Interlancompu

get more video functionalities in Asterisk.
