I think you have to set the IP of the PC with the TFTP client as the tftp server on the phone. I seem to recall something about the name of the file as well. Again, it's quite foggy as I did this about a year ago so sorry I can't be of more help. I remember getting those packet fails too until I set up all the parts correctly. The documentation for the phone should have all the info. If I could figure it out I'm sure you can.
