I've used Vitelity.net since they were 'iax.cc' and have no complaints. 
Cheap ($1.49 a month for a DID + $0.0137 a minute) and reliable.

Back in the day, you called Customer Service and the guy that answered the 
phone would click a few keys on his keyboard and the problem went away.

Now you fill out a service request web page, but they're still reasonably 
responsive.
