I need a straight origination/termination provider on a per minute charge
plan. I would like to avoid a monthly subscription-based provider.

 have you considered teliax.com ?

 check your numbers for LNP at the bottom left.

 I have been playing with voip for only about a month, but
 no complaints with teliax svc so far.
