gain tuning should be the last resort.
Before that:
1. Make sure you have the proper value for opermode (if this is the US,
the defalt, FCC, is probably OK).
2. Use fxotune to further adjust the FXO modules.
3. Make sure you actually use an echo canceller.

You don't cancel echo on the FXS(?) modules? Why not? Echo should be
cancelled as close as possible to the source: this will make it the most
effective. What happens when you call from an FXS port to a VoIP phone?

You use KewlStart and still have to reort to busydetect?
